A fertility ultrasound — sometimes called a fertility scan or pre-pregnancy ultrasound — is designed to assess the health of the female reproductive organs. It provides vital information about the uterus, ovaries, and endometrium, all of which play a key role in conception.

This scan is often recommended as part of a fertility health check, either before trying to conceive naturally or in preparation for assisted conception treatments such as IVF.

What This Scan Includes

Every fertility scan appointment includes:

  • Detailed ultrasound examination of the uterus, ovaries, and endometrium
  • Assessment of ovarian volume and antral follicle count (AFC) for ovarian reserve, indicating a woman’s potential fertility and response to treatments like IVF

  • Digital and printed report provided on the same day
  • Digital images sent securely and available to download for your records

Pelvic ultrasounds can be carried out in two ways:

Trans-abdominal scan: Gel is applied to the lower part of the abdomen and an ultrasound probe is used to assess the pelvis. This requires a full bladder to improve image quality.

Trans-vaginal scan (TVS): A small probe is inserted into the vagina to provide a more detailed view of the uterus and ovaries. This option is only performed with your consent.

** IMPORTANT INFORMATION **

A trans-vaginal ultrasound gives a much clearer picture of the uterus, endometrium, and ovaries compared to a trans-abdominal scan. It can provide more accurate assessment of ovarian volume, fibroids, or cysts, and is particularly useful in fertility assessments. This closer, more detailed view can detect subtle changes that may not be visible otherwise, giving added reassurance and supporting early diagnosis.

  • Drink 1-2 pints of water an hour before your appointment to ensure a full bladder
  • Wear comfortable, loose-fitting clothing
  • A trans-vaginal scan gives the most detailed information, so be prepared that this may be recommended
  • Do not empty your bladder until the trans-abdominal part of the scan is complete

Allow approximately 30 minutes in clinic (to include forms and bladder emptying), with scan time ranging between 5-20 minutes depending on scan type.

During a fertility scan, ultrasound is used to examine:

  • Ovarian reserve — by counting antral follicles and measuring ovarian volume
  • Endometrium — thickness and pattern, which are important for implantation
  • Uterus — assessing for structural abnormalities, fibroids, or polyps
  • Ovaries — checking for polycystic ovarian appearance or cysts
  • Cycle monitoring — follicle tracking and endometrial development at different stages of the cycle

3D ultrasound provides a more detailed assessment of the uterus compared with standard 2D imaging. It allows the uterus to be viewed in the coronal plane, giving a complete picture of the shape and contour of the uterine cavity, which is particularly important when assessing fertility.

This advanced imaging helps evaluate whether the cavity has a normal appearance or if structural differences may affect implantation or pregnancy outcomes. Using 3D ultrasound, we can assess:

  • Uterine cavity shape and contour
  • Congenital uterine variations such as septate, bicornuate, or arcuate uterus
  • The presence and position of fibroids or polyps that may distort the cavity
  • Endometrial cavity symmetry and volume if requested by your gynaecologist
  • Accurate coil (IUD) position if present
  • Relationship between the uterine lining and surrounding muscle

Because implantation occurs within the uterine cavity, understanding its exact shape and structure can provide valuable reassurance or identify findings that may benefit from further gynaecological review. 3D ultrasound enhances diagnostic confidence while remaining non-invasive, painless, and performed during your routine pelvic fertility assessment.
